Medical Records

Release forms are available for download (English | Spanish) or by calling (602) 506-6018. Release forms are also available at the Medical Records office, located at 1645 E. Roosevelt Street in Phoenix. Office hours are Monday through Friday, 7:30 AM to 5:00 PM.

Completed release forms can be faxed to (602) 506-5079. Faxed or mailed forms must be accompanied by a Notary Form and a legible copy (front and back) of your photo ID.

**Please note: If the name and date of birth on your photo ID do not match the name and date of birth on your record, you will not be able to get copies of your medical record or lab results**

Walk-ins for copies of medical records are accepted Monday through Friday between 9:00 - 11:00 a.m. and 2:00 - 4:00 p.m.
