Welcome to the MCDPH Office of Vital Registration Medical Certifier resource and information page. We have developed this website to aid you and your staff in preparing the necessary information for a decedent’s death certificate. The site is divided into four tabs: Communication, Training & Handbooks, Forms, and Statute & Rules.

The Communication tab contains pertinent letters and other communication from government agencies.

The Training & Handbooks tab contains resource links from the CDC. Additional training resources will be added at a later date.

The Forms tab contains all applicable forms needed to submit medical certification in the State of Arizona.

The Statutes & Rules tab contains searchable PDFs of both Arizona Vital Records’ Statutes and Rules related to death.
