How do I apply for WIC?
*Call a
clinic
close to you to make an appointment.
*Bring all family members applying for WIC to the appointment, along with one item for each type of the following documentation:
1. Identification: bring ID for each person in the family applying for WIC.

DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF

Driver's license

Paycheck stub

Military ID

School ID card
-
Passport
-
Birth Certificate
-
Child shot/immunization record

School ID card

Hospital crib card
-
Hospital record
2. Address (where you live): bring a document that has your name and current address on it.

DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF
-
Driver's license
- Paycheck stub
- Utility bill
- Rent reciept
- Lease agreement
- Signed letter from the person you are living with that has the address written on it
3. Income: bring proof of your entire household income.

DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF

Paycheck stubs for everyone in the household who works

W2 or tax forms

Unemployment letter/notice

Signed and dated statement from your employer showing your income
4. Shot/Immunization Record: please bring if you have a child less than 2 years old.
*Please note that a DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF counts for all three types of documentation. However, if you are not enrolled in any of these programs, you may still qualify for WIC.
