License your Dog
Dog owners must complete a license application within 15 days of vaccination. Residents new to Maricopa County and new dog owners must purchase a license within 30 days. A penalty will be assessed to owners who don’t apply within the required timeline. Penalty fees are as follows: $2 per month for sterile dogs. $4 per month for unaltered dogs.
It’s easy to license your dog. You can download a form right here and print out a copy of the dog license Application/Renewal Form. You can also buy a dog license in person at any of our locations during normal business hours. The term of the license is based on the month and day of the most current rabies vaccination recorded at MCACC. (You can get a rabies vaccination from your veterinarian or at any of our three locations.) Licenses are valid for no more than one calendar year.

Acceptable forms of proof that your dog has been spayed or neutered is a sterilization certificate from the veterinarian that completed the surgery or examined your dog to verify the surgery or a previous license information showing spay/neuter status.
If your dog loses his license, you may purchase a replacement tag for $4.00. You can send us a check with a short note stating that your dog lost his tag and include the tag number from your original receipt. If you can’t find the receipt, please include your name, address, phone number and the dog’s name and description. You can also come into one of our centers to get a new license.

If you adopted a currently licensed dog from its owner, you do not have to purchase a new license. Simply apply for a transfer of ownership at any of our centers or by mail. To apply by mail, send a written and signed request, the original license receipt and a $4.00 transfer fee to the address above.
