Environmental Services Department Recommendations
A plumbing system shall installed to preclude backflow of a solid, liquid or gas contaminants into the water supply system.
Except for ware washing machines with a properly vented floor drain trap and ware washing and culinary sinks:
1. Direct connections may not exist between a sewage system and a drain originating from equipment in which food, equipment and or utensils are placed.
Except as specified in 9 CFR 308.3(d) for fire fighting, a person may not create a cross connection by connecting a pipe or conduit between the drinking water system and a non-drinking waster system.
Multi use food-contact surfaces shall be smooth, free of breaks, open seams, cracks, chips, inclusions, pits, sharp internal angles and corners.
Multi use food-contact surfaces shall have smooth welds and joints unless simple disassembly is possible and/or it concerns beverage syrup lines or tubes.
A hand washing facility shall be located: (A) To allow convenient use by employees in food preparation, food dispensing and ware washing areas; and (B)In, or immediately adjacent to, toilet rooms.
Hand washing facilities shall be functional at all times.

THIS ESTABLISHMENT FAILED TO MEET MINIMUM STANDARDS TO QUALIFY FOR A PERMIT, A CEASE AND DESIST ORDER WILL BE REQUESTED. CALL FOR REINSPECTION WHEN ALL VIOLATIONS ARE CORRECTED AND YOU'RE READY TO OPEN (ALLOW 2-3 DAYS FOR SCHEDULING).
THIS ESTABLISHMENT HAS MET THE REQUIREMENTS TO SIGN A STIPULATION AGREEMENT AT THIS TIME. CONTACT THE DEPARTMENT LEGAL LIAISON AT (602) 506-6621 TO SIGN A STIPULATION AND ORDER.
