This notice outlines important information for FSA participants!!!
This message applies to you if you are currently enrolled in the Mariflex Flexible Spending Accounts for health care, limited use, and/or dependent care.
As previously communicated to you in an email message of 3/5/09, beginning April 1, 2009, Maricopa County is changing the Mariflex Flexible Spending Account (FSA) administrator from ASI to ADP. During the transition period from March 15 through Mach 31, claims could not be submitted for reimbursement to ASI. Claims can be submitted to ADP on or after April 1, 2009.
Employees enrolled in the health care FSA began receiving their new debit cards at their home address yesterday. Please look for a white window envelope with the ADP logo showing through the window. The debit card is provided to you at no cost. It can be used as a payment convenience at your medical and dental offices, hospitals, health care clinics, pharmacies and vision care providers to purchase the same eligible items (including over-the-counter items) and services for which you submit paper-based claims for reimbursement. When using the debit card, you will not be required to file a claim. Please be sure to keep your receipts as the FSA Administrator may request verification of the items or services purchased with the debit card.
There will also be paper forms for your use for both the health care and the dependent care FSAs.
