Important Information About Your September 10, 2014 Paycheck
Benefit premium deductions are based on pay period end dates, not pay dates. This means that months with three pay period end dates will not have benefits-related premium deductions taken on the paycheck that corresponds with the third pay period end date. In the month of August there were three pay period end dates: August 3rd, August 17th, and August 31st. For this reason, your September 10, 2014 paycheck (which is for the pay period that ended August 31st) will not have benefit premium deductions taken. This includes contributions to a health savings account and flexible spending accounts, and deductions to pay down any benefit premium arrears balance you may owe.

Dependent Care Flexible Spending Account Deadline Approaching
The deadline to file claims for reimbursement from a Dependent Care Flexible Spending Account is quickly approaching. Take action now to avoid forfeiting your balance!
If you were enrolled in the Dependent Care Flexible Spending Account for Plan Year 2013-2014, there is still time for you to file claims for reimbursement of eligible dependent care expenses incurred between July 1, 2013 and June 30, 2014. For active employees, the final claims filing deadline is August 31, 2014.* Failure to file claims by the deadline will result in you forfeiting the balance remaining in your account. Exceptions to this timeframe are not allowed, so please do not delay in filing your claims. Click here for the link to the Dependent Care Flexible Spending Account claim form.

Are Your Beneficiary Designations Up-to-Date?
Now that the new Benefit Plan Year has begun, it is an ideal time to check your life insurance, retirement, and health savings account (if you are enrolled in the UnitedHealthcare HDHP) to ensure your beneficiary designations are up-to-date. A beneficiary is the person or entity you designate to receive a benefit in the event of your death.
Life Insurance Beneficiary Designations
Maricopa County provides you with Basic Life and Basic Accidental Death and Dismemberment insurance. You also have the option to purchase Additional Life and Additional Accidental Death and Dismemberment insurance.
To check your beneficiary designations for life insurance:
- Log onto the ADP portal
- Select “Welcome” from the Benefits Menu
- Navigate to the Beneficiary Summary page
Retirement System Beneficiary Designations
The County also contributes to your retirement through the Arizona State Retirement System (ASRS) or Public Safety Personnel Retirement System (PSPRS).
To check your beneficiary designations for retirement:
Health Savings Account Beneficiary Designations
If you are enrolled in the UnitedHealthcare HDHP with H.S.A. Plan, you will need to add your beneficiary information to your Health Savings Account.
a. Log onto http://www.myuhc.com
b. Click on “Claims and Accounts” in the top menu bar
c. Click on “Health Savings Account” under “Claims and Accounts”
d. Click on “ Manage Your HSA”
e. Click on “ Manage your Profile” under Self Service
f. Click on “Beneficiary Designation” under Personal Information
g. Complete the beneficiary designation information, and sign it with a digital signature
h. You will need to complete a paper “Designation of Beneficiary” form and mail/fax it to Optum Bank only if you are married and your primary beneficiary is NOT your spouse.

The Take Care Health Center (480.347.4791) and Walgreens Pharmacy (602.283.9925) conveniently located on the second floor of the Administration Building provides onsite pharmacy, health care, and laboratory services Monday - Friday, 7:30 a.m. - 4:00 p.m. The center is closed daily for lunch from 1:00-1:30 pm.
