The Adobe Acrobat version of the Notification Form can either be typed in online and then printed OR the application can be saved, then typed in and printed. You will not be able to save the information you have typed into the form, you can only print it (unless you have a full version of Adobe Acrobat). TO SAVE APPLICATION FORM:
For Internet Explorer users - Right click on the application you want to download; choose "Save Target As"; save pdf document to your computer.
For Netscape users - Right click on the application you want to download; choose "Save Link As"; save pdf document to your computer.

A NESHAP Notification shall be postmarked or delivered to the NESHAP coordinator at least ten business days before beginning regulated asbestos renovation activity or before beginning any demolition activity of a regulated NESHAP facility, even when no asbestos is present.
