Current patients may have their medical records transferred to a new provider or may pick up their personal records.
The MCDPH clinic is open from 8 a.m. to 5 p.m. Monday - Friday until May 8th. For information, call (602) 506-6635.

Do you recommend any other providers for my family planning needs?
We have complied lists of providers in our local community who will continue to provide you and your family excellent family planning service. Please review these two provider lists and contact them today.
