Considerations for current clinic patients:
 
How to obtain your personal medical records?

Current patients may have their medical records transferred to a new provider or may pick up their personal records.   The MCDPH clinic is open from 8 a.m. to 5 p.m. Monday - Friday until May 8th.  For information, call (602) 506-6635.
 
May I have my medical records transferred to another provider and is there a fee?
 
Current patients may have their medical records transferred to a new provider.  The MCDPH clinic is open from 8 a.m. to 5 p.m. Monday - Friday until May 8th. For information, call (602) 506-6635.  There are no fees for this service.

Do you recommend any other providers for my family planning needs?
 
We have complied lists of providers in our local community who will continue to provide you and your family excellent family planning service.  Please review these two provider lists and contact them today. 
 
Access to family planning is crucial to the health of any community.
It has been a privilege to serve our community with this program for these many years.
We are sorry for any hardship that this may cause our clients.
