School Health Assessment and Planning

The Office of School Health and Wellness Initiatives uses the national School Health Index (SHI) to help Maricopa County schools create, improve, or better sustain a healthy school environment for both students and school staff. Built on CDC’s evidenced-based guidelines for the Coordinated School Health Model, the SHI does the work for you, helping your school to identify and prioritize which school health programs are most likely to be effective in promoting healthy behaviors at your school.

How it Works

Participating elementary, middle or high schools receive a $1,000 grant for the school and a $1,000 stipend for the designated school coordinator, who is trained to help convene a School Health Advisory Council (SHAC), a group of school staff, parents, students and community members who will lead your wellness efforts.

SHAC members will come together to complete the SHI. This assessment will help you identify opportunities to improve the health and wellness of your staff and students. The SHAC will develop an action plan to address areas they want to improve.

When you successfully complete this first phase, your school is eligible to begin your School Health Improvement Plan (SHIP) the following school year. Approved SHIP schools will receive $2,500 to fund sustainable activities in their plan.

Ongoing Support

With the help of a self-assessment process and a customized action plan, our team will support your school as they work to improve policies and programs. Our team can provide support, such as:

  • A prevention specialist who can provide training, answer questions and connect you with additional Public Health services.
  • Online school health hub to help locate community partners and resources
  • Health fair and classroom tools including displays, models, DVDs and other educational opportunities
